Appointments

In order to minimize the wait time, patients are seen by appointment only.  Emergencies, of course, will be worked into the schedule as needed.  When you call for an appointment you will be asked the nature of the visit.  This information is important to determine the length of the visit.  If you are unable to keep an appointment we request at least 24-hours notice of cancellation.  This courtesy allows us to give your appointment time to another patient.  A missed office visit will result in a $45 fee that is not covered by your insurance company.   Please keep in mind that multiple no shows will result in a termination from the practice.

Insurance

Bethlehem Family Healthcare contracts with most major insurance carriers.  Due to the constant changes in insurance plans and policies, it is your responsibility to know your health coverage.  We encourage you to confirm with your insurance company if we are a participating provider.  Policies and plans vary even within the same carrier; therefore, if you have any questions regarding your deductible, copay or coverage, please contact your insurance company.  Please remember that insurance is meant to reimburse the doctor for services obtained by the patient and is not a substitute for payment.

Per your contract with your insurance company, we are required to collect your co-pay at the time of service.  It is your responsibility to pay any deductible amount, co-pay, or any balance not paid by your insurance.  We may ask you to reschedule your appointment if you do not pay your co-pay at the time of service.

As a courtesy, we will bill your health insurance.  Sometimes your insurance will request information directly from you; it is your responsibility to provide them with requested information.  Patients who do not follow the guidelines set up by their insurance will be responsible for denied services.

Self-Pay

We offer a time-of-service discount to patients who do not have insurance.  The full amount is required at the time of service.  Additional charges may apply if you receive in-house labs, injections, or procedures.  We will do our best to estimate the cost of the visit based on complexity.

Important Reminders

  • Please bring your insurance card and ID each time you come to the office to see the physician.
  • If there is no verifiable insurance, services are provided on a “self-pay: basis.  This means that services must be paid in full prior to seeing the doctor.  For your convenience, payments can be made by cash, check or credit card.  We accept MasterCard, Visa, and Discover Card.
